With Disease Management, we optimize outcomes and target indices for commonly encountered disease states (e.g., hypertension & diabetes) while incorporating Medication Therapy Management & Targeted or Comprehensive Medication Reviews in conjunction or separately.
Remote Patient & Clinical Outcomes Monitoring through digital therapeutics are integral to these. With Chronic Condition Management, we leverage the same to rectify, mitigate &/or prevent medical conditions and complications in patients with two or more chronic conditions (or as defined by CMS).

We assess all medications being taken (including supplements and OTC drugs) for safety, efficacy and appropriateness, and if unavoidable risks are present, we employ strategies to mitigate those risks, especially in patients 65 and older.

We work hard to ensure you get your prescribed medications in a timely and hassle-free manner, while facilitating affordability for all (Medication Access & Cost-Containment through Patient Assistance Programs, Coupons, & Formulary Management).

Additionally, we provide access to high quality behavioral health medication management. The application of evidence-based integrated care models allows behavioral health clinicians to be embedded in and become regular and routine contributors to primary care teams. This includes our board-certified psychiatric pharmacists who work in close collaboration with psychiatrists to manage medications within an integrated primary care Behavioral Health program.

We have a proven data-driven track record of raising HEDIS/STARS medication-related measures from substandard to desirable points (4 or 5 stars). Examples of the measures we impact are all the Medication Adherence, Care of Older Adults (COA), Statin Use in Persons with Diabetes (SUPD), etc.

We provide After-hours Care including visits with our Physicians and other Providers and Pharmacists to prevent Urgent or Emergency Room visits, worsened health, and unnecessary health care expenses, while facilitating optimal health outcomes, consistency of care and optimized transitions/follow-up. Among others, this also contributes to CMS requirements for Chronic Condition Management.

We partner with global health organizations to facilitate Medication Access in low access regions, create Pharmaceutical Care Models for Global & Region-Specific populations, and provide Pharmacotherapy & Disease Management Training Programs, etc.
